Disclosure Scotland Document Guidelines

Upload Identity Documents

Please upload at least two forms of identity documents. If possible, at least one should be photographic. The documents uploaded should provide confirmation of your Name, Date of Birth and Current Home Address

Examples of Proof of Name & Date of Birth

  • Passport
  • National ID Card
  • Driving Licence (see note below)
  • Birth Certificate

Examples of Proof of Current Address

  • Utility Bill
  • Bank, mortgage or credit card statement
  • Vehicle registration document
  • Council tax statement
  • Driving Licence (see note below)

Note: If you upload a Driving Licence then you do not need to upload a second document as the Driving Licence will contain all of the details required.
